Despite the development of modern dentistry and the widespread use of fluoride-containing toothpastes, the prevalence of caries and dental hypersensitivity continues to increase. Among the reasons for increased tooth sensitivity are non-carious lesions (wedge-shaped defects and erosions) (Fig. 1 a, b), exposure of tooth roots due to periodontopathy (Fig. 2), professional whitening (Fig. 3) and, of course, caries (Fig. 4).
Traditionally, to eliminate sensitivity, drugs containing fluoride ions, desensitizers and toothpastes aimed at reducing sensitivity are used [1]. However, the effect of most of them is temporary and after some time sensitivity appears again. Due to the fact that increased tooth sensitivity interferes with adequate tooth brushing and promotes the formation of a mature biofilm, the issue of eliminating sensitivity is directly related to the prevention of inflammatory periodontal diseases and caries.
Currently, the most effective drug for eliminating or reducing dental hypersensitivity, regardless of its cause, is hydroxyapatite. A number of manufacturers introduce hydroxyapatite in nanoform (nano-HAP) into toothpastes for home and professional use. Toothpastes with nano-HAP have become widespread in Japan. Recent research by the well-known company Sangi has made it possible to develop and introduce Apadent toothpaste, the active ingredient of which is medical nano-HAP, whose high preventive effectiveness is based on the fact that about 97% of tooth enamel and 70% of dentin contain hydroxyapatite. Therefore, when applied topically, hydroxyapatite nanoparticles contained in toothpastes and gels help restore the mineral density of the surface and subsurface layers of enamel, ensuring the smoothness of its surface and preventing the development of caries [2]. Interacting with dentin, nano-HAP clogs dentinal tubules, incl. forming HAP-protein complexes. This completely prevents vibrations of the dentinal fluid, so increased tooth sensitivity is significantly reduced or disappears. Analyzing scanning electron microphotographs of dentin after bleaching and treatment with a paste containing nano-HAP, R. Takikawa et al. (2007) found that nano-HAP particles not only clog the openings of the dentinal tubules, but also form a pronounced protective layer [7]. H. Kawamata et al., studying the surface of teeth treated with nano-HAP, fully confirmed this, finding not only complete occlusion of open dentinal tubules, but also a protective layer 1.8 microns thick located on the surface of dentin [3]. W. Browning et al. Based on the analysis of data obtained using a visual analogue scale, it was found that nano-HAP quickly eliminates tooth sensitivity that occurs after whitening procedures [2].
However, the effect of nano-HAP as a means of caries prevention is of greatest interest. In addition to the direct effect on the surface and subsurface layers of demineralized enamel, it was found that the caries-preventive effect of nano-HAP can be associated, incl. with its ability to sorb polysaccharides produced by Streptococcus mutans and lipoteichoic acid. This means that it is nano-HAP that reduces the adhesive ability of dental plaque and the ability of microorganisms to aggregate [4]. An important fact should be considered that toothpastes based on HAP have the lowest abrasiveness and practically do not damage the surface of the teeth, because nano-HAP is applied in the form of a gel or extremely fine particles [5].

Clinical example No. 1. Using professional toothpaste “Apadent Pro” to eliminate tooth sensitivity
Paste with nano-HAP was used in eight patients with wedge-shaped defects, in cases where the patients temporarily refused filling due to the small size of the defect (Fig. 5 a and b). To prevent the defect from deepening, patients were recommended to use an atraumatic cleaning technique and soft toothbrushes. In addition, it was recommended to refrain from taking sour juices; if it is impossible to refuse sour juices, rinse your mouth immediately after eating and start brushing your teeth no earlier than an hour later. The effectiveness of treatment with professional paste Apadent Pro was assessed based on the determination of the Schiff index, based on identifying the threshold of tooth sensitivity using an air jet according to Schiff [6]. The test was carried out as follows: air from the gun of the dental unit was supplied perpendicular to the cervical surface of the tooth from a distance of 1 cm for a second. Tooth sensitivity was assessed using the following criteria: 0 – no reaction; 1 – feeling of discomfort, but the patient does not insist on stopping the test; 2 – discomfort accompanied by a request to stop the test; 3 – severe pain reaction with pronounced motor reactions aimed at immediately stopping the test. The Schiff index was determined before treatment, immediately after treatment and a week later. If discomfort returned, patients were asked to come back for an appointment.
The results of the study showed that even a single treatment of the area of wedge-shaped defects with Apadent Pro gave a lasting clinical effect. The Schiff index before treatment was 2.37±0.09. Immediately after treatment it decreased to 0.8±0.24. When determining the Schiff index after a week, we obtained 1.0±0.7. Within a month, none of the patients came for re-treatment. The patients were recommended to use Apadent toothpaste for daily use.
Clinical example No. 2
To evaluate the remineralization potential of Apadent Pro professional paste, we used it in patients with caries in the white spot stage. The patient complained that due to the rough surface of the white spots on teeth 43 and 44, he could not effectively remove plaque (Fig. 6).
Indeed, upon examination, the presence of caries in the white spot stage of teeth 43 and 44 was revealed. When dried, the surface does not shine, is rough upon probing, and the cervical area is covered with plaque. The algorithm for remineralizing therapy with nano-HAP included the removal of plaque from the tooth to be treated. Then the surface was dried and treated with antimicrobial treatment. For this, we used an ozone-air mixture generated by the ProZone device for 6 seconds, because treatment of the tooth surface with ozone gas prevents the formation of biofilm by strains of S. mutans and Lactobacillus acidophilus [3]. Exposure to the ozone-air mixture helped stop the leakage of gingival fluid or exudate of the gingival pocket. Apadent Pro professional paste was applied to the dried surface, and the procedure was carried out three times. For home use, Apadent paste was recommended and the mandatory use of plaque control by staining. When examined a week after completion of remineralization therapy, the color of the spots remained white, but its surface became smooth. The patient reported that now high-quality cleaning of teeth 43 and 44 is not difficult.
