Installation of dental implants in patients with HIV infection and the likelihood of infection during implantation

The health and appearance of our teeth determine not only how other people perceive us, but also our self-confidence. It is for this reason that dentists are constantly developing methods thanks to which we can enjoy a flawless smile and live without complexes. Unfortunately, not everyone can use this method of restoring an ideal dentition due to the invasiveness of the procedure and the resulting contraindications.

Is it possible to get implants for HIV?

Immune system diseases suppress the body's ability to detect and destroy pathogens. A defective immune system is unable to resist the attack of pathogens. The ability of tissues to naturally regenerate also decreases. The risk of dental implant failure in patients with a history of complications increases by 80%.

The emergence of one-stage technologies has significantly reduced the list of contraindications to implantation - today immunodeficiency is not an obstacle to implant-based prosthetics.

Absolute and relative contraindications

Absolute contraindications are those that make the procedure impossible. These include:

  • mental disorders characterized by inappropriate behavior of the patient;
  • diseases of the blood, hematopoietic organs;
  • tuberculosis in the acute stage;
  • connective tissue pathologies;
  • malignant tumors on the head, neck;
  • slowing down blood clotting, complicating stopping bleeding and wound healing;
  • chronic alcoholism, drug addiction;
  • diseases of the immune, central nervous system;
  • impossibility of anesthesia due to allergic reactions;
  • congenital anomalies of the jaw that prevent the implantation;
  • age under 18 years;
  • renal failure.

Relative diseases include diseases and conditions that can be eliminated and then implanted:

  • poor nutrition;
  • venereal diseases;
  • endocrine pathologies;
  • diseases of the temporomandibular joint that interfere with opening the mouth;
  • anemia;
  • viral infections;
  • smoking;
  • regular stress, exhaustion;
  • diseases eliminated by dental treatment;
  • insufficiency of bone tissue.

Relative contraindications include chronic diseases that require regular monitoring by doctors (compensated diabetes mellitus, HIV, hepatitis).

When is implantation performed?

Dental implantation for HIV and AIDS is possible with normal CD4 cell counts and the absence of negative symptoms. Preparation for surgery involves a complex of laboratory and instrumental studies. In addition to the standard set of tests, a patient with immunodeficiency donates blood for the number of CD4 lymphocytes and viral load. Implantation is performed when CD4 cell counts are relatively high and there are no adverse symptoms.

  • With a CD4 count of more than 500, the risk of complications is the same as in patients without immunodeficiency.
  • If the CD4 cell concentration is 450-400, the risk of artificial root rejection increases, but surgery is possible.
  • If the CD4 count is 350 or less, surgery cannot be performed. In this case, antiretroviral therapy should be continued. Once stable CD4 counts are achieved, the doctor will consider dental implantation.

With a very reduced immune status, the risk of pathogenic microflora growing along the implant is very high. That is, the likelihood of re-implantitis and, as a consequence, rejection of the artificial root increases many times over.

Is it possible to hide your HIV status from the dentist?

In theory, a person has the right not to disclose details of his medical record and talk about his HIV status. In practice this is far from the case. According to the laws of the Russian Federation, the patient must warn the doctor that he is HIV-infected. The implantologist must be aware of the possible risk, because he carries out treatment in accordance with the general state of health - he chooses the optimal method, implantation system, and effective antibiotic therapy. In addition, immunodeficiency is included in the list of restrictions for surgery. Therefore, tests for infectious diseases, including HIV, are included in the mandatory preoperative examination. So you won’t be able to hide your immune status from your doctor.

Why Roott implants are the best solution for HIV infection

ROOTT implants are made of biocompatible titanium with an antimicrobial surface. When the sterile packaging of the implantation system is opened, the titanium reacts with oxygen. This promotes the formation of a special hydroxyapatite film on the surface of the implant. Since hydroxyapatite is the main component of bone tissue, the chance that the jawbone will not reject the titanium root increases many times over. The polished surface of the implant prevents the accumulation of bacterial plaque in the cervical area, minimizing the risk of developing peri-implantitis.

Obstacles to implantation in the upper and lower jaws

The main obstacle to implantation is atrophy of the bone of the upper or lower jaw. After the root of a natural tooth is removed, the alveolar process becomes thinner over time. If you install an implant into atrophied bone tissue, over time it will deteriorate even more, and the metal part of the artificial tooth will be exposed. To prevent complications after implantation, jaw bone augmentation is performed.

Problems when installing an implant in the upper jaw are caused by sinusitis - an inflammatory process in the maxillary sinus of the nose. The infection that causes the disease weakens the immune system and complicates the operation. Since the implants are located near the sinuses, pathogenic bacteria can spread to the jaw tissue. The inflammatory process that develops leads to rejection of the metal part of the implants.

Where to place implants

The ROOTT clinic in Moscow is one of the few dental centers in Russia that specializes in one-stage implantation with immediate loading. Our implantologists have been working with the basal protocol for many years. The presence of HIV or AIDS in a patient is not a valid reason for refusing dental implantation. Our specialists will carefully study your medical history and offer the best option for restoring your dentition. Thanks to the ability to combine protocols in one system, treating patients with a complicated medical history is not a problem for us.

Can a doctor refuse implantation?

Refusal is possible in the following situations:

  1. The implantologist does not know modern treatment protocols and considers HIV an absolute contraindication to implant prosthetics.
  2. The doctor is afraid of infection because he will have to come into contact with the patient’s blood.
  3. CD4 cell count is too low.
  4. For medical reasons (in addition to immunodeficiency, there are a number of complications).

Positive HIV status is not a reason to refuse medical care . According to Article 124 of the Criminal Code of the Russian Federation, a doctor who evades his duties without a valid reason is liable, including restriction of freedom.

Removable prosthetics

Removable dentures are an easy way to replace missing teeth, regardless of the state of the immune system. The main disadvantages of the method:

  • Does not prevent bone atrophy;
  • restores chewing function by only 60%;
  • removable orthopedic systems cannot be used in the presence of opportunistic diseases of the oral cavity (aphthous stomatitis, exfoliative cheilitis, Kaposi's sarcoma, etc.).

With the help of removable structures, you can replace both several missing teeth and an entire row of teeth.

Mini implantation

Lightweight mini-implants are implanted through micro-punctures. They can be installed even in the narrowest area of the bone without pre-building it. They are characterized by rapid healing and securely hold the removable denture in the mouth. They are not intended for heavy chewing loads, therefore they are not suitable as a support for a permanent denture.

Mini-implantation and removable prosthetics are a good alternative during antiretroviral therapy.
As soon as CD 4 cell counts are stabilized, basal implantation is recommended.

Which implantation method is best to choose?

Currently, dentists are increasingly recommending one-stage dental implantation to patients. Because with this technique, teeth are restored not only very quickly, but also with a predictable result - especially if you need to build up bone or perform a sinus lift. In case of HIV infection, emphasis is also placed on Immediate Load implantation (with immediate load) - teeth are restored in 1-5 days, the prosthesis is aesthetically pleasing and reliable. And potential complications are mitigated thanks to modern drugs for rapid tissue restoration. Let's consider which one-step protocols are suitable for patients diagnosed with HIV:

  • to restore only the lower jaw: All-on-3 or Trefoil technique,
  • for the restoration of all teeth on any jaw: the All-on-4 or “All on Four” concept for mild inflammation and mild bone atrophy, the All-on-6 technique for moderate atrophy and more severe inflammation,
  • for the restoration of all teeth in case of acute bone atrophy: basal implantation, which is also called Basal Complex and Strategic Implant, is very effective here,
  • To restore all teeth in the upper jaw with severe atrophy, zygomatic implantation is used,
  • to restore a segment of teeth: 3-4 implants are installed, which can immediately be loaded with a fully functional bridge,
  • to restore 1 tooth: after installing the implant, an abutment is fixed on top, and a temporary shortened crown is placed on it. It is removed from the bite while the implant takes root (chewing and biting with it is not recommended). In the meantime, gum is forming around the abutment and crown. Permanent prosthetics can be done in a few weeks (it will be done quickly and bloodlessly), i.e. This is not immediate, but earlier prosthetics.

It is worth noting that any implants will not be suitable here - you need to use special improved models. They will be discussed below.

Advantages of one-stage implantation

Why is it better to choose one-stage implantation for HIV infection? Taking into account weakened immunity and potential problems with healing, survival, and exacerbation of inflammatory processes, it is better to minimize the number of surgical stages and the level of stress. One-stage protocols have an advantage here, since in 1 day and 1 operation it is possible to remove decayed teeth and place implants and dentures on top.

And in case of bone atrophy (with periodontitis, periodontal disease, etc.) in 99% of cases there is no need to first build up the bone or do a sinus lift. All this can be combined with implantation or the implants can be installed at an angle in suitable areas of the jaw. There is no need to install a gum former or undergo plastic surgery on the gums to correct their level. The less and less often the tissues are injured, the more favorable it is for the immune system.

Benefits of immediate dentures

According to patients, the main advantage of immediate dentures is that the teeth are “installed” immediately - and with such a denture you can immediately chew food. According to implantologists, immediate prosthetics is beneficial because the bone recovers and regenerates faster under chewing load. That is, engraftment occurs efficiently and quickly. It is recommended to choose prostheses with a metal or fiberglass frame. They absorb overloads better, and the pressure on all implants is distributed evenly.

Special products for tissue regeneration

During implantation, doctors often use additional drugs, thanks to which the bone is restored faster, and the wounds heal in a few days. Also, these drugs protect against the appearance of massive edema and hematomas. For example, if there is a lack of bone at the implant site, bone material is added to it (Bio-Oss, Kasios, ISIOS+, TCP Dental, etc.). To prevent the bone chips or block from moving or falling out, it is covered with a special collagen membrane, which will resolve on its own after a few weeks (for example, Creos xenoprotect from Nobel Biocare). Biogels that stimulate regeneration can also be used (Emdogain from Straumann, for example).

Another good assistant for the “implantologist” is a laser. It replaces a scalpel when cutting gums, instantly stops bleeding, tones the gums and stimulates their healing.

Possibility of infection in the dentist's chair

The risk of contracting HIV or AIDS in the dentist's chair is zero, since all manipulations are carried out under conditions of absolute sterility. Anti-epidemic measures and sterilization at the RUTT clinic are carried out in accordance with the AntiAIDS-AntiHEPATITIS program, regardless of the type of treatment performed. Only disposable consumables are used (sterile gloves, masks, napkins, disposable instruments). After use, consumables and disposable instruments are disposed of.

How to recover quickly after surgery

Patients with HIV status need to agree on a list of medications in advance (for rehabilitation after implantation, it is issued by the dentist) with their treating infectious disease specialist. Typically, antibacterial and antiviral medications are taken at different times of the day, rather than together. Implantologists often recommend starting antibiotics before implantation (take the first tablet 2 hours before), but such a regimen must be prescribed by a doctor.

In addition to antibacterial drugs, dentists prescribe painkillers, antihistamines, and vitamins (for example, vitamins PP and C). To speed up the healing of wounds, you should take healthy foods rich in vitamins and microelements - yoghurt, cottage cheese, lean meat, fish, herbs, fresh vegetables and fruits (not hard ones).

Questions and answers

Is it possible to completely implant the upper and lower jaw
Good day. I wanted to know if complete implantation of the upper and lower jaw is possible, since I have the absence of almost all teeth due to the genetic disease anhydrotic ectodermal dysplasia. I've been wearing dentures since I was 6 years old.

Hello! Currently, at the SIMPLADENT center on Dmitrovskaya there is a promotion for complex basal implantation of both jaws. The cost of implantation, taking into account all manipulations (diagnosis, orthopantomogram, installation of implants, anesthesia, installation, adjustment of prostheses, subsequent preventive examinations) and prostheses within the framework of the promotion is 450,000 rubles. The promotion is valid only until July 30, 2011. Since you have been using removable dentures for a long time, the bone may have severely atrophied. You need to attend a consultation with an implantologist to assess the condition of the bone and determine which method of implantation and prosthetics is right for you. As for the disease you indicated, it does not affect the condition of the bone, so implantation in your case is theoretically possible. We invite you to a free consultation with a doctor at one of the center’s clinics. The doctor will examine the oral cavity, tell you exactly what measures are needed to prepare for basal implantation, and give directions for tests. Because there are a number of contraindications for any type of implantation, you can find them in the “Indications and Contraindications” section. Sincerely, SIMPLADENT++ Patient Support Center 7 (495) 789-35-82

Hello. Is full implantation possible for periodontal disease? It’s difficult to say the stage of the disease. Visually - loose gums, protrusion, loosening and partial loss of teeth. Therapeutic treatment gives a temporary positive result.

Hello. Is full implantation possible for periodontal disease? It’s difficult to say the stage of the disease. Visually - loose gums, protrusion, loosening and partial loss of teeth. Therapeutic treatment gives a temporary positive result.

Hello! Gum diseases are not an absolute contraindication to dental implantation; in each case, the possibility of installing implants is determined individually. The use of root-shaped implants in your case is not recommended, since due to problems with the gums they may not take root. In order to definitively answer whether BOI implantation is right for you, a direct examination and photograph of the jaw is necessary. Sign up with us for a free consultation, the doctor will take a picture (orthopantomogram, done free of charge as part of the consultation), and then inform you about the possibilities of dental restoration in your case. We also recommend that you familiarize yourself with the list of absolute contraindications to implantation (these are blood diseases, some chronic diseases, etc., which may cause implant failure). Read more about contraindications on our website in the section Indications and contraindications for dental implantation. Sincerely, SIMPLADENT Patient Support Center + + 8-800-333-53-41

Other questions

When are dental implants installed?

Tooth loss can result from a variety of circumstances, including illness or neglect, accidents, or bad habits. The situation in which a person loses teeth is always associated with discomfort. Sometimes even more important than aesthetics is a purely functional issue.

Dental implants are installed:

  • when a tooth is lost as a result of carious changes or trauma
  • in people with undeveloped dental buds
  • when some or all teeth are missing

How is HIV diagnosed in Moscow?

There are two tests to diagnose the virus: preliminary ELISA and the most accurate immunoblot. The accuracy of ELISA is about 90%. It is recommended to carry out it 3-6 months after contact with the virus, then it gives maximum accuracy. The usual ELISA test is based on a blood test, but there are also rapid tests that help obtain information based on urine or saliva. Such texts are purchased exclusively at the pharmacy (in no case on the Internet!), since it is necessary to use officially approved products.

If the rapid test gives a positive result, you need to go to an infectious disease specialist yourself. In such a situation, as well as when ELISA in a blood test gives a positive result, the patient is prescribed an immunoblot. Its reliability is already 99.9%. Depending on the diagnosis, the diagnosis is made either on the basis of two repeated tests or a combination of both. The analysis is rechecked and only after this a diagnosis can be made. This is necessary in order to exclude false positive results that may occur during the diagnostic process.

Important: the test does not show how HIV is transmitted in a particular situation - that is, you can determine the route of infection only by analyzing your own actions.

Recommendations for oral care

In the postoperative period, it is necessary to make applications with healing and/or anti-inflammatory gels, ointments, and perform oral baths with antiseptic solutions. Suitable medications are prescribed only by a doctor. In the first days, you should carefully care for wounds and stitches. After each meal, oral baths are performed, because... You can't rinse yet. And after 3-5 days you can start using a toothbrush and toothpaste, for example, Parodontax, and rinsing.

If inflammation begins under the implant, what to do?

Don't try to treat it yourself. Let's run to the doctor. He must make a decision, take a picture and, of course, carry out cleaning or prescribe stronger antibacterial agents.

Without the help of a doctor, severe inflammation can occur, which can lead to serious complications and even death.

Believe me, implant rejection is the most harmless loss; you only lost time, money and slightly limited your chewing function. It all passes without a trace.

Signs of Dental Implant Rejection

Understanding that implant rejection has begun will not be a problem; you may experience the following signs:

  • Pain;
  • Edema;
  • Perceived feeling of pressure;
  • Temperature;
  • Redness of mucous membranes;
  • Deviation of the installed implant.
