A wisdom tooth is the last large molar on the edge of the jaw that begins to erupt in adulthood. The eruption of this tooth often causes discomfort, as well as various complications, such as:
- inflammation of periodontal tissues,
- inflammation of the trigeminal nerve.
The wisdom tooth sometimes deviates to the side, as it is supported by the neighboring tooth on only one side.
Unfortunately, this tooth (also called the “eight”) often only harms adjacent teeth, complicates oral hygiene, is often destroyed due to caries and becomes the cause of inflammatory processes. In such cases, dental surgeons at the clinics of the German Implantology Center recommend the removal of wisdom teeth, performing such operations atraumatically and at a highly qualified level

What are the indications for removal? And what are the problems with wisdom teeth?
- They are difficult to clean due to poor accessibility, so caries develops very quickly on the “eights”. Caries often becomes complicated - inflammation of the gums begins
- There is a risk of tooth decay between the wisdom tooth and the adjacent tooth. It will probably be necessary to treat the neighboring teeth as well.
- There are injuries to the tongue and cheek due to improper placement of wisdom teeth.
- If wisdom teeth are positioned incorrectly, this can lead to deformation of the dentition.
- Possible cheek biting. Chronic cheek biting is often associated with wisdom teeth
- Also, these teeth are one of the causes of crunching and pain in the joint (TMJ) when opening the mouth and when chewing.
Can third molars be treated?
Wisdom teeth, like any other teeth, can be treated. But only if they erupt correctly, do not threaten the health and position of the rest of the dentition, and there is confidence that the treatment will be successful. If the specified conditions are met, it makes sense to preserve the “eight” for the following indications:
- If the sixth and seventh teeth are sick or severely damaged and must be removed, in this case the wisdom tooth is left as “the best of the worst” in order to provide the patient with at least some conditions for chewing, and the doctor - the basis for future prosthetics.
- If the six and seven have already been removed and it is necessary to preserve the abutment tooth for installation of prosthetic structures.
- In the presence of an antagonist tooth, completely healthy and subject to preservation.
In case of curved roots that complicate endodontic intervention, severe tissue destruction, dystopia or retention, no competent dentist will undertake to treat the “eights” - he will definitely recommend removal.
Do I need to remove uncut “eights”?
Not all people develop their “8s” by the right age. Sometimes they remain in the anlage, that is, retained (not erupted). When to remove them:
- if on an x-ray we see that the wisdom teeth abut adjacent molars (seventh teeth). There is a risk that wisdom teeth will “eat” into an adjacent tooth inside the bone. And then you will have to remove 2 teeth.
- The pressure of the “eights” on neighboring teeth can cause caries of the root of the “sevens” - our main chewing teeth.
- If the wisdom teeth are large and there is no place for them in the oral cavity. There is a high risk that they will collide with the dentition, and a crowded position of the front teeth will appear.
- During implantation next to unerupted figure eights. It is worth removing the tooth so that it cannot damage the implant in the future.
The essence of the technique

Rehabilitation after complex wisdom tooth removal

Preparatory stage

- Avoid physical activity and visiting the sauna and swimming pool.
- Perform oral hygiene daily. When brushing your teeth, do not touch the post-operative area with a brush.

Complications

Then, using surgical instruments (forceps, elevator, chisel, etc.), the wisdom tooth is removed in parts;
- treatment of the surgical wound and socket in the alveolar process with antiseptics;
- suturing the gums with special surgical threads and (or) packing the socket.
After some time, a clot forms in the hole, which prevents infection of the wound and speeds up its healing. If necessary, especially in the presence of an inflammatory process, the doctor prescribes a course of broad-spectrum antibiotics.
