Is it possible to give arsenic during pregnancy? Dental treatment with arsenic during pregnancy

During pregnancy, the expectant mother needs to listen more carefully to her body. Oral problems during pregnancy are not uncommon. Teeth and gums react very sensitively to changes occurring in a woman’s body; enamel is more susceptible to destruction and can react sharply to environmental influences. Many women think that dental treatment during pregnancy is impossible. In fact, dental treatment during pregnancy is an important and necessary procedure. After all, the condition of the teeth and the unborn child largely depends on the condition of the mother’s teeth.

Arsenic in the tooth during pregnancy: composition of arsenic paste

“Pure” arsenic is an extremely aggressive substance. A dosage exceeding 5 - 50 mg can kill a person. To minimize all possible risks, arsenic in homeopathic doses is added to special pastes. The remaining mass fraction of the product is allocated for the following components:

  • anesthetic substance that relieves toothache (dicaine, novocaine);
  • an antiseptic substance that destroys harmful microbes in the cavity of a diseased tooth (thymol, camphor);
  • astringent tannin, which increases the period of use of the paste.

The finished drug is equipped with a convenient dispenser, which helps the doctor correctly calculate the dosage. After placing arsenic paste into the tooth, a temporary filling is placed. When treating single-rooted teeth, the application time for such a filling is 24 hours; for any other units, this figure increases to 48 hours. The maximum possible and safe duration of action of the arsenic substance is strictly limited to 72 hours.

We remind you that the use of paste with arsenic during pregnancy is rather an exceptional case. This method of pain relief is necessary if the expectant mother is intolerant to other anesthesia drugs, and the treatment of a diseased tooth cannot “wait”.

Methods of anesthesia in the treatment of pulpitis

Perhaps no area in practical medicine is developing as rapidly as dentistry. Modern methods of treating carious teeth make it possible to completely eliminate the use of arsenic pastes.

A wide range of painkillers of different pricing policies, produced in the form of injection solutions, solves the problem of pain relief in dental procedures of any complexity. The most common drugs for local anesthesia are available to every modern dentist: Ultracain, Ubistezin, Septanest, Scandonest, Mepivastezin and others.

However, not all therapeutic actions can be performed without resorting to arsenic paste. For example:

  • inability to perform anesthesia due to health reasons (tendency to hypertension, previous myocardial infarction);
  • individual intolerance to strong analgesics;
  • heart failure;
  • bronchial asthma;
  • autoimmune diseases;
  • the impossibility of expanding and cleaning curved or tartar-filled canals.

When, for various reasons, it is not possible to relieve high sensitivity with the help of anesthetics, the doctor uses devitalizing pastes containing arsenic anhydride.

Nerve removal during pregnancy

Even women who have exemplarily tidied up their teeth at the stage of pregnancy planning are sometimes faced with the need to treat them during the difficult but joyful period of waiting for the baby. What can we say about the ladies for whom the conception of a child was a surprise for which they did not have time to properly prepare?

Removal of the tooth nerve during pregnancy is necessary to treat pulpitis or periodontitis. Both diseases are a complication of caries, which for some reason was not detected or treated in a timely manner.

Any caries should be treated immediately, without delay. And the best prevention is a regular preventive examination by a dentist and professional hygiene specialist twice a year.

Should a tooth be pulped during pregnancy?

Many patients know that the nerve has to be removed if pulpitis begins. It is always accompanied by pain, a reaction to cold and hot, often a rise in temperature, and night pain.

Is it possible to depulp a tooth during pregnancy? Any dentist will say: “Yes.” If pulpitis begins, then removal of the tooth nerve during pregnancy is necessary.

Pulpitis is an acute inflammatory disease. It is impossible to stop it without removing the source of inflammation. Pulp pain is one of the most severe, and therefore it is impossible to endure and wait for the tooth to “calm down” under any circumstances. In addition, even if the tooth aches and hurts periodically, delay in treatment will lead to the development of periodontitis - an even more formidable and serious disease.

Drug poisoning

Arsenic and all its compounds are poisons. You cannot die from placing a temporary filling with the substance, but side effects can still threaten your health.

Main manifestations of poisoning:

  • nausea, vomiting;
  • diarrhea;
  • bright metallic taste in the mouth;
  • sharp pain in the abdominal area;
  • CNS depression;
  • at the final stage, convulsions, paralysis appear and the death of the victim occurs.

What happens if arsenic stays in the tooth for a long time? The action of a strong toxin destroys not only the pulp, but also surrounding tissues.

With prolonged use of the paste or an incorrectly calculated dose of the drug, the following consequences may occur:

  • darkening of dentin;
  • poor circulation and swelling at the filling site;
  • gum burn;
  • periodontitis;
  • periodontitis;
  • necrosis of periosteum and bone tissue;
  • poisoning of the entire body.

Nerve necrosis

In modern medicine, temporary filling pastes are actively replaced with safer medicinal substances. Arsenic poisoning during dental treatment occurs when the basic rules and dosage are violated.

The following conditions are contraindications to this method of nerve removal:

  • allergy to paste components;
  • childhood and pregnancy;
  • incorrect channel position;
  • kidney pathologies;
  • root damage;
  • high intraocular pressure.

How to avoid nerve removal

As mentioned above, pulpitis is a complication of caries. Normally, the crown of the tooth is covered by enamel, and underneath it is a layer of dentin. In the cavity of the tooth there is a pulp chamber in which the neurovascular bundle is located - this is the pulp.

With the development of caries, enamel and dentin are affected, and a focus of inflammation appears. As soon as it passes into the pulp chamber, pulpitis occurs, this is inflammation of the neurovascular bundle.

The development of caries occurs gradually: the spot stage (damage to the enamel), then superficial caries, then deep. This is how the primary process occurs. The second option is secondary caries. It develops on a previously treated tooth, maybe in a place where a filling has fallen out or directly under an old filling.

The best way to avoid nerve removal during pregnancy is regular preventive examinations, professional hygiene before and during pregnancy, and timely treatment of caries.

I fixed all my teeth and here I go again...

Our doctors regularly hear from pregnant patients: “I had all my teeth treated before pregnancy, and now I need to put fillings in again. How so?"

During pregnancy, the whole body experiences stress:

  • changes in hormonal status;
  • global decrease in immunity;
  • exacerbation of chronic diseases;
  • increased pathogenicity of microflora;
  • the appearance of allergies;
  • manifestation of diseases that the patient did not suffer from before pregnancy.

The condition of the oral cavity also changes during pregnancy. Caries appears more often and progresses faster. Good hygiene requires much more effort, and the condition of the gums worries every second pregnant patient. All this increases the likelihood of developing pulpitis during pregnancy.

When is the best time to remove a nerve during pregnancy?

The best time for dental treatment during pregnancy is considered to be the second trimester. During this period, the placenta maximally protects the fetus from negative influences, the formation of organs and systems is already behind us, and the risk of miscarriage is minimal.

However, pulpitis is usually accompanied by acute pain, which is an indication for immediate intervention. In addition, delay invariably leads to periodontitis - a more severe and formidable disease when inflammation from the tooth cavity spreads to the tissues surrounding the tooth.

Therefore, removal of the nerve of a tooth is carried out for pregnant women at any stage: severe pain syndrome is a strong stress that negatively affects the condition of the woman and child, and significantly increases the risk of miscarriage.

How is nerve removal performed for pregnant women?

Treatment of pulpitis is usually carried out in two visits. The doctor does not use x-rays, relying on objective and clinical symptoms of the disease, such as the intensity and nature of pain, reaction to temperature stimuli, and external signs of inflammation.

The dentist collects a detailed medical history, conducts examination, percussion, and palpation. In case of a questionable picture, the doctor can additionally use electroodontometry for differential diagnosis.

In general, the process of depulpation itself differs little from the treatment of pulpitis outside pregnancy. Under local anesthesia, the doctor removes pathologically altered tooth tissue, expands and processes the canals, and removes the neurovascular bundle. The canal is sealed and a temporary filling is placed. After a few days, a permanent filling can be installed.

Features of root canal treatment in pregnant women

  • Potentially painful procedures are performed under local injection anesthesia. The anesthetic used to remove the nerve does not contain adrenaline and is approved for use by children and pregnant women.
  • When removing a nerve during pregnancy, X-rays are not used. Other methods are used for diagnosis, and a control image is recommended to be taken after childbirth.
  • To treat root canals during pregnancy, the doctor does not use drugs containing formaldehyde, arsenic, lidocaine and other toxic substances.
  • During the treatment process, the doctor takes short breaks so that the pregnant patient can rest, change her position, and walk around.
  • If there is post-filling pain, the patient is prescribed paracetamol, explaining that other painkillers are not recommended in her situation.

Toothache

Toothache, in its intensity, is one of the most powerful sensations in the human body. In terms of the level of discomfort, it can only be compared with the pain syndrome when stones come out of the renal pelvis. Discomfort felt in the tissues of the tooth and outside the tooth surface is called dentalgia. The pain is caused by destructive processes occurring inside the tooth.

When toothache rapidly increases and is also accompanied by additional destructive processes, the risk of tooth loss is high. Symptoms of inflammatory processes progressing in dental tissues are:

  • sharp pain when chewing;
  • sharp pain when sweet and sour, hot and cold come into contact with tooth enamel;
  • throbbing pain when lying down;
  • bleeding from the gums;
  • redness of the gums;
  • swelling of the gums and jaws;
  • radiating pain to the ear or maxillary sinus;
  • purulent discharge from the open pulp;
  • increase in body temperature.

Only a qualified specialist can provide full assistance to a diseased tooth. At home, you can only reduce pain and relieve increased swelling with the help of painkillers and rinsing the mouth with various medicinal solutions.

In cases with pregnant women, the situation is much more complicated. Taking painkillers can negatively affect the development of the embryo, especially in the first trimester of pregnancy, because the placenta is not yet fully formed. Drugs that reduce the pain threshold of sensitivity have a vasoconstrictor effect, which is extremely dangerous in the first months of fetal development.

In any case, diseased teeth must be treated; temporary relief of pain symptoms will not make it possible to eliminate the cause of their occurrence. Moreover, the destructive process that has begun in the tissues of the teeth cannot be stopped in this way.
